If you have had to go into hospital to receive treatment for surgery or illness and are due to be discharged, United We Care offers a thoughtful Hospital to Home Support Plan to ensure a smooth and safe transition. We are here to offer the support and encouragement needed to help regain confidence and independence in the comfort of home. Our carers will assist in re-learning routines and daily tasks, ensuring that each step is met with attention and patience. We are here for as long as you need, providing emotional support and practical assistance to make sure you feel settled and supported at home.
At United We Care, we know the importance of feeling connected and supported. Whether you need help with daily tasks, emotional support, or companionship, we are here to provide the care and warmth deserved. Together, we can help live your life with dignity and joy through meaningful connections.
Additionally, we offer support with managing your medication routine and ease the transition back into your home after a hospital stay. Our services are as individual as you are, whether it’s short-term support following an illness or long-term assistance for as long as you need it.
We understand how important it is for family caregivers to take care of themselves. That is why we offer flexible respite care, so you can take a much-needed break, knowing your loved one is in the safest hands or allowing us to take care of the household whilst you rest. Whether you need 7 days or more we will work with you to create a care plan that fits your family’s needs. At United We Care, we treat our clients like family, and we are here to help you and your loved one continue living life in the most fulfilling way possible surrounded by warmth, respect, and all the support they deserve.
United We Care understands the importance of a good night’s sleep, especially for those who are more vulnerable due to illness or disability. Restful sleep is essential for overall health and recovery, and a lack of it can affect both physical and mental wellbeing. We know that a wakeful or restless night can impact daily life, and even lead to a weakened immune system, potentially requiring additional care. That is why we offer our Night Care Service to ensure that you are comfortable, relaxed, and able to rest well through the night with the assurance that help is at hand.
Our carers are here to support you during the night, helping you maintain a peaceful and restful sleep environment. Whether it is assisting with mobility, helping you get settled for bed, or providing reassurance if you wake during the night, we are here to ensure that your night is as calm and restful as possible. Our objective is to help you regain and retain your independence, so you can live life to the fullest, feeling well rested and rejuvenated. With our compassionate care, you can rest assured that your needs will be met, giving you the comfort and peace of mind you deserve.
We offer two types of night care services: Waking Night Care and Sleeping Night Care, both of which are designed to provide you with the right level of support during the night. Our services are flexible, ensuring that your care plan is tailored to suit your specific needs. Whether you need assistance throughout the night or simply someone nearby for peace of mind, we can adjust your support as needed.
If your night-time needs are less frequent, but you still want the comfort of knowing someone is nearby, Sleeping Night Care might be a better fit. Your carer will stay overnight in your home, sleeping in a nearby bedroom, and be available to assist if needed. This service is ideal for those who may feel uneasy being alone during the night, offering peace of mind and ensuring you can sleep soundly. You can call on your carer up to three times during the night. If your needs change and you require more frequent support, we can easily upgrade your care to the Waking Night Care plan, ensuring you receive the necessary assistance.
If you need frequent support during the night, Waking Night Care is the ideal solution. A dedicated carer will stay with you throughout the night, usually starting their shift in the evening and finishing in the morning though we can adjust the exact timing to fit your schedule. Your carer will stay awake and be available to assist with any needs that arise, such as helping you to the toilet or assisting with repositioning in bed. During your sleep, your carer can take care of domestic tasks, such as cleaning, laundry, or vacuuming, and even prepare breakfast or ensure your medical needs are ready for the morning. We understand that interruptions at night can be difficult, and our goal is to make this process as seamless as possible, so you can get the rest you need.
We understand that your needs can fluctuate, and our support plans are designed to adapt to these changes. Whether you require more care at times or less, we will work with you to ensure your care is always right for you. Our goal is to provide the comfort and reassurance that you need to live your life with independence and peace of mind.
